Invisalign Attachments: What They Are and Do They Hurt?
Short answer
Do Invisalign attachments hurt?
No. Attachments are bonded to the tooth surface without drilling or injections, and fitting takes around 20 to 40 minutes. The teeth may feel tight for a day or two because aligners grip harder once attachments are on, and the bumps can feel rough on your lips at first.
Attachments are one of the parts of Invisalign treatment that surprise people most. You expect clear trays; you are then told that small bumps will be added to some of your teeth. They are a normal and important part of modern aligner treatment. Without them, a smooth plastic tray would simply slide over the teeth for many types of movement, particularly rotations, root movements and vertical changes.
| Question | Short answer |
|---|---|
| What are they made of? | Tooth-coloured composite resin, the same family of materials used for white fillings |
| How many will I have? | Anything from none to around 20, depending on your plan |
| When are they fitted? | Usually at the first or second aligner appointment |
| Does fitting hurt? | No; no drilling or injections |
| How long do they stay? | Until the end of active treatment |
| How are they removed? | Polished off in one visit |
| Do they damage teeth? | No, when fitted and removed properly |
What do Invisalign attachments do?
Each attachment is a precisely shaped bump that gives the aligner something to push against. Different shapes have different jobs:
- Rectangular attachments help tip, rotate and move roots, and add retention.
- Optimised attachments are shaped by the software for specific movements, such as rotating a canine or extruding a front tooth.
- Bevelled attachments help with vertical movements like intrusion and extrusion.
- Precision bite ramps are small raised areas behind the front teeth used to open a deep bite. See overbite treatment for adults.
- Button cut-outs are used with elastics for bite correction.
The number and position are planned digitally as part of your treatment. Our guide to how Invisalign works explains the planning process.
How are attachments put on?
- The teeth are cleaned and dried.
- A mild etching gel is applied to small areas of enamel to help the resin bond.
- A template tray, which looks like an aligner, is filled with composite in the shape of each attachment.
- The template is placed over the teeth and the composite set with a curing light.
- The template is removed and excess material is cleaned away.
The whole process usually takes 20 to 40 minutes.
Do attachments hurt?
Fitting does not hurt. Afterwards:
- Aligners feel tighter because they grip more firmly.
- Teeth may ache for a day or two as movement starts.
- Lips and cheeks can feel the bumps when aligners are out; this settles within a week.
- Taking aligners out takes a little more practice. Start at the back teeth and work forward.
Our guide does Invisalign hurt? covers what to expect over the whole of treatment.
Can people see Invisalign attachments?
They are colour-matched to your teeth, so most people will not notice them at a normal distance. Attachments on front teeth are more visible when aligners are out and in bright light, and they can pick up surface stains from coffee, tea, red wine or curry. Brushing after eating and drinking only water with aligners in helps keep them discreet.

| Do | Avoid |
|---|---|
| Brush around each attachment twice a day | Biting directly into very hard foods with front teeth |
| Brush before putting aligners back in | Drinking coloured or sugary drinks with aligners in |
| Use a soft brush and fluoride toothpaste | Picking at attachments with nails |
| Keep aligner wear to about 22 hours a day | Long gaps without aligners |

What happens if an attachment falls off?
Attachments can occasionally chip or come away, often after biting something hard or removing aligners at an awkward angle. If it happens:
- Keep the aligner in; it will still work for most movements.
- Let the practice know which tooth it was on.
- Check the aligner still fits snugly.
- The attachment is usually replaced quickly using the template.
A missing attachment for a few days usually makes little difference. Over weeks, it can mean a tooth stops tracking with the plan.
How are attachments removed?
At the end of treatment, the orthodontist removes attachments with a fine polishing bur. The process is painless, takes around 15 to 30 minutes, and leaves the enamel smooth. Retainers are usually made at the same visit; see retainer costs.
Do attachments damage your teeth?
No, when fitted and removed correctly. The etching affects only a microscopic surface layer and the enamel is polished afterwards. Some people notice a faint outline for a few days until the tooth surface rehydrates.
{{CTA}}Do other aligner brands use attachments?
Yes. Most aligner systems use attachments, sometimes called engagers or buttons. Our comparison of Spark aligners vs Invisalign covers how the brands differ, and Invisalign cost in the UK and London explains what treatment typically costs.
Do attachments change how long treatment takes?
Attachments do not add time; they help teeth follow the plan, which keeps treatment on schedule. When attachments are missing or worn down, teeth can stop tracking, and that is what extends treatment because refinement aligners are needed. For typical durations by case type, see how long Invisalign takes.
Can I choose not to have attachments?
You can ask, and in simple cases the plan may need only a few. For most cases, though, removing attachments from the plan would mean some movements cannot be achieved, the result would be less predictable, or treatment would take longer. We will show you exactly where attachments are planned on your digital simulation before you start, and explain what each one does.
Attachments and eating
You take aligners out to eat, so attachments are exposed at mealtimes. Most foods are fine. To reduce the chance of an attachment chipping, avoid biting directly into very hard foods with your front teeth, such as whole apples, crusty bread rolls, hard sweets and ice. Cut hard foods into pieces and chew with your back teeth. Brush or at least rinse thoroughly before putting your aligners back in, because food trapped around attachments is held against the teeth under the trays.
Attachments and teeth whitening
Whitening during treatment is possible with some aligner plans, but the composite used for attachments does not whiten. If you whiten with attachments on, small areas under the attachments may look slightly different once they are removed. Many people choose to whiten after attachments are removed, often using their retainers as whitening trays, so the colour is even.
Common questions about attachment shape and number
The number of attachments is not a measure of how difficult your case is; it reflects which movements are planned. Rotations of rounded teeth, such as canines and premolars, often need an attachment. Moving roots, levelling teeth vertically and holding anchorage for elastics also call for them. Your orthodontist can add, change or remove attachments during refinements if teeth are not tracking as planned.
Will I need attachments on my front teeth?
Often only a few, and sometimes none. Many plans place most attachments on canines and premolars, which are less visible in a normal smile.
