# Does Invisalign Hurt? What to Expect

Invisalign does not usually hurt in the sense of sharp pain, but most people feel pressure and tenderness for one to three days each time they start a new set of aligners, and the first week of treatment is normally the most noticeable. Over-the-counter pain relief, cold water and soft food are usually enough to manage it.

## Why does Invisalign feel sore?

Aligners move teeth by applying gentle, continuous pressure. That pressure causes a small inflammatory response in the ligament and bone around each tooth, which is part of how the bone remodels and lets the tooth move. The same process happens with fixed braces. The tenderness is a sign the aligner is working, and it fades as the teeth settle into the new aligner.

Each aligner is designed to move teeth only a fraction of a millimetre, which is why the discomfort is usually mild and short-lived. It tends to be strongest when you put a new aligner in and when you take aligners out and put them back after eating.

## How long does Invisalign pain last?

For most people, the first week is the most uncomfortable, with aching teeth and tender gums for two to four days. After that, each new aligner usually causes pressure for one to three days, often less as treatment goes on and you get used to it. Many people say that by the third or fourth set they barely notice the change.

If soreness lasts more than a week, gets worse rather than better, or is focused on one tooth, contact us. It may mean an aligner is not seating properly or an edge needs smoothing.

## Does Invisalign hurt more than braces?

Most people find aligners less uncomfortable than fixed braces. There are no brackets or wires to rub the cheeks, no wires that can poke out, and the pressure is spread over smaller, more frequent steps. With fixed braces, tenderness after each adjustment can be more intense and can last a few days longer. ## Do Invisalign attachments hurt?

Bonding attachments does not hurt. The teeth are cleaned, a template is used to place small amounts of tooth-coloured composite, and it is set with a light. No injections are needed. Afterwards, attachments can feel rough against your lips and cheeks for a few days, and new aligners can feel tighter where they clip over them. This usually settles within a week. ## Does IPR hurt?

IPR (interproximal reduction) is gentle polishing between teeth to create a little space. It is done with fine strips or small discs and removes only a fraction of a millimetre of enamel. Most people feel vibration and pressure rather than pain, and anaesthetic is rarely needed. Some people notice mild sensitivity for a day or two afterwards.

## How can you make Invisalign less painful?

- **Change to a new aligner at bedtime.** You sleep through the first few hours of pressure.

- **Take simple pain relief** such as paracetamol or ibuprofen if needed, following the dose on the packet and checking it suits you.

- **Keep the aligners in.** Taking them out for long periods to relieve soreness makes the next fitting more uncomfortable and slows treatment.

- **Use chewies.** Biting gently on these small soft cylinders helps the aligner seat fully and can ease the pressure.

- **Drink cold water.** It can soothe sore gums.

- **Eat soft food for the first day or two** of a new aligner: pasta, soup, eggs, yoghurt, fish.

- **Use orthodontic wax** on any rough edge until we can smooth it.

- **Do not cut or file the aligners yourself.** Tell us instead.

## Does Invisalign hurt your gums?

Gums may feel tender in the first few days, and the edge of an aligner can occasionally rub. Good cleaning helps, because food trapped under aligners irritates gums and raises the risk of decay. If an edge is cutting into your gum, contact us and we can adjust it. Bleeding gums that do not settle with good brushing can be a sign of gum inflammation, which we will want to check.

## Can Invisalign cause headaches or jaw pain?

Some people notice mild jaw ache, particularly when elastics are added to correct the bite or if they clench on their aligners. This usually settles within a few days. If you grind your teeth at night, mention it at your consultation, as it can affect how aligners wear and feel. Persistent jaw pain, clicking or locking should be checked.

## When should Invisalign pain worry you?

- Sharp pain in one tooth that does not settle within a few days.

- An aligner that does not fit, leaving a gap between the aligner and the teeth.

- A cracked or broken aligner with sharp edges.

- Swelling, a bad taste or signs of infection around a tooth or gum.

- Pain that wakes you at night.

These are uncommon, but they can mean something needs adjusting.
